Student Referral Process

Personal, academic and/or financial problems can place students at risk of failing a course. The purpose of the Student Referral Process is to provide early identification and intervention for students at risk for a variety of reasons. This Process is designed to support both students, faculty and staff. The Student Referral Form will be submitted electronically to the individuals as indicated on the form or the Blue Healer Support Team for follow-up.

Student Referral:

  • Student Code of Conduct Violations
  • Academic Integrity Violation
  • BCHS Mental Health Practitioner Concern
  • Academic Student Support Concern
  • Other

To activate the Student Referral Form, these steps should be followed:

  • Anyone can identify students at risk and fill out a Student Referral Form
  • When a student “at risk” is identified, it is recommended but optional to inform the student than a Student Referral will be initiated.
  • An Student Referral Form is completed on the student outlining the concerns identified. The completed form is filled out electronically and submitted to the Blue Healer Support Team as indicated on the form.
  • The appropriate representative will contact the student via email or phone when necessary
  • A member from the Blue Healer Support Team will initiate the next steps.  These actions can include:
    • Contacting the student via messaging, email, or phone
    • Arranging a meeting with the student's academic advisor
    • Requesting additional information or clarification from the person who submitted the student referral
    • Referring the student to additional services
  • Individuals who complete a Student Referral Form will receive a confirmation email that the document was received.  This process ensure a coordinated and comprehensive response to each student referral.
